DNA Groups - Permission from Donor's Manager/Proxy

Background

Many of the same folks help out with both the Reeves DNA Project and the wiki at The Reeves Project (TRP). This works well, but does cause TRP one significant issue. The facilities for the Reeves DNA Project are provided by FamilyTree DNA (henceforth FtDNA) and as administrators there we have to abide by their Group Administrator Policy. This places various obligations on the Reeves DNA project admins, including maintaining the anonymity of DNA Donors beyond the DNA Project. This means the admins can't take the information you've shared with us and the others within the Reeves DNA Project at FtDNA and post it at The Reeves Project.

Now whilst we'd be the first to support confidentiality and security by default, the real value of a DNA project is only unlocked if the participating members are prepared to open up a little and share. And we do that sharing principally within TRP.

So we need you (as the manager of a DNA sample) to opt-in and give explicit permission to The Reeves Project (TRP) to associate your name and that of the DNA Donor with the DNA test kit number. In the absence of explicit permission, we simply use their initials which are deemed acceptable to maintain anonymity.

Giving Permission

As the manager of the DNA test kit, we ask that you send an e-mail with a subject line of DNA Proxy Permission.   If you manage multiple Y-DNA test kits, please note we need a separate e-mail for each kit.   In the body of that e-mail we need you to answer the following questions ...
(1) What is the FtDNA Kit Number you manage?
(2) What is the full (legal) name of the DNA Donor?
        (TRP's Admins need this information in case the donor subsequently approaches TRP directly.)
(3) If the DNA Donor is already a TRP member, what is their TRP Membership ID?
(4) Have you consulted with the DNA Donor and have they given their explicit consent to TRP to name them as the Donor of the above FtDNA Kit?
(5) What name should TRP show for the Donor?
        (This does NOT need to be their full name. It should at be least their initials and surname. Their preferred given name plus surname would be best.)
(6) What is your full (legal) name?
(7) Do you, as the manager of this DNA sample, give consent to TRP to name you as the Interested Party of the above FtDNA Kit?
(8) What name would you like TRP to show for yourself?
        (Again, this does NOT need to be your full name.)
(9) If you are already a TRP member, what is your TRP Membership ID?
(10) Was the DNA Donor's most recent Reeves ancestor female?
        If you answer YES here, their results will be included in DNA Group XX.
(11) If Yes, what relation is their most recent female Reeves Ancestor to the DNA Donor?
        For example is she their Mother, Grandmother, GGM, &c, Aunt, Great Aunt &c.
        Additionally and purely optionally, please feel free to name their most recent female Reeves Ancestor.

So we can cross check, it is important you send your email granting TRP permission from the same e-mail address you have registered against the above FtDNA Kit.
